Refer a Patient
The Pediatric Feeding Program provides individualized, comprehensive care to children with and without autism who have complex feeding disorders. We strive to empower and give hope to families through interdisciplinary care to ultimately improve their child’s meals, nutrition, and development.
How to Refer a Patient
If you are unsure about whether to refer your patient, please call the Pediatric Feeding Program at 206-987-0008.

We are accepting new referrals.
New evaluation wait times are 12 to 18 months. Families can ask to get on our cancellation list and/or register for our Feeding Foundations Class (max wait time is 4 weeks) by calling us at 206-987-0008.
Referral requirements
Please submit a referral that is complete and attach the referring provider triage form. We will close the referral if this form is not completed within 30 days of referral.
About the referring provider triage form: We use this form to clinically triage patients and may redirect the patient to another specialty or subspecialty if there is a more appropriate place to see them. We may call to follow up with any questions or recommend additional referrals based on this triage.
Please include in your referral:
- Service/specialty clinic requested
- Reason for referral: what is the clinical question for the specialist?
- ICD-10 diagnosis – required
- Visit type:
- New patient consult, transfer of care, second opinion, or return visit/ongoing care
- ALL relevant clinical documents
- Clinic notes
- Medication history
- Growth charts/curves
- Lab reports
- Imaging and diagnostic reports (Images can be uploaded to PowerShare)
- Previous specialty evaluations
- Patient’s full name, DOB, sex, address, guardian contact information and insurance
- Referring provider’s name, phone, fax and the referral coordinator’s email address so that we may contact you if additional information is needed
- If an interpreter is needed
- Any known barriers to performing a successful telehealth (video) visit with the family
- Referring provider triage form
Please note:
- We serve children with and without autism.
- We treat children from age 1 to 15. We will accept referrals for persons over 15 years of age with intellectual disability.
- For ages and conditions that we do not treat, please see refer your patient to the appropriate programs below:
- Infants: Infant Feeding Program
- For children who are not medically or behaviorally complex and have never had feeding treatment: Occupational Therapy
- Swallow concerns: Speech and Language Services
- Eating disorders (with or without autism): Eating Disorders Recovery Program
- Eating concerns secondary to psychosis or schizoaffective disorder: Psychiatry and Behavioral Medicine
