How to Refer a Patient

Please note that while we encourage referrals from their PCP, patients may self-refer to Orthopedics and Sports Medicine.

We encourage use of our New Appointment Request Form (PDF or DOC) or submitting a referral via EpicCare Link. Please complete these forms in their entirety to ensure timely processing of your patient’s referral. If your clinic uses its own referral form, please ensure all fields listed in the NARF are included in your documentation.

eConsult Information

An eConsult is a non-urgent asynchronous electronic consultation request initiated by a community provider to a Seattle Children’s specialist, submitted using EpicCare Link. It allows community providers to receive timely, efficient care guidance on lower-complexity and data-oriented clinical questions that may not require an in-person evaluation.

Orthopedics and Sports Medicine offers eConsults for the following conditions:

  • Mass concern
  • Infant hip concern
  • Lower extremity concern
  • Upper extremity concern
  • Spine concern

For more information on eConsults, including helpful resources and instructions on obtaining access to EpicCare Link, please visit our EpicCare Link and eConsults page.

What Your Patients Can Expect

  • Because Orthopedics and Sports Medicine does not require a referral, patients may call us to schedule their appointment.
  • If a patient has been referred to our program, they should expect a call from us when we are ready to schedule, or they can proactively call to schedule an appointment which we will match with the referral once processed. Patients who are referred to Orthopedics and Sports Medicine and have opted in to text notifications from Seattle Children’s will receive text messages as their referral works through our queue, ultimately letting them know we are ready to schedule their appointment.
  • Orthopedics and Sports Medicine utilizes MyChart Fast Pass, which will notify families if an earlier appointment becomes available so they can be seen even sooner by our providers.
