What to Expect
Preparing for a Genetic Medicine Clinic Visit
2 weeks before your appointment:
- Send us medical records from clinics, hospitals or other healthcare providers outside of Seattle Children’s. We already have your child’s records from Seattle Children’s providers.
- Records we need:
- Lab reports with recent results
- Recent clinic notes
- that we have requested, like reports from X-rays or
- Documents related to your child's condition, such as your family's history with the condition or a note stating that a parent has been diagnosed with a genetic condition
- Genetic test results from your child or other family members if they have had testing
- Send us these records or have your child’s primary care provider send these records at least 2 weeks before your appointment. Your provider can upload images to PowerShare or send them by mail or by fax to 1 of the following programs:

Before your appointment
In addition to the records you send us 2 weeks before your appointment, we ask some families for more information:
- When we schedule your child's visit, we will let you know if your child will need some lab work done before their first visit.
- We may ask you to track your child's symptoms and diet using a 3-day food intake form. Please fill it out and bring it with you to the visit.
